NW Speech Therapy Financial Policy
Any co-pay/co-insurance/unmet deductible and/or private pay amounts owed to NW Speech Therapy are due at the time that services are rendered unless previously discussed and an alternative payment schedule is agreed upon. If at any time an outstanding balance is incurred, the balance is to be paid in full by the last session of the month in which the balance was incurred. For families provided with invoices, the balance is to be paid upon receipt of invoice. Any balance that has not been paid in full will be assessed a late fee of $5 on the 14th day of the next month or from the date of the invoice, and an additional $10.00 for each WEEK (7 days) past due. Accounts past due in excess of 45 days will be sent to Columbia Collectors Agency and therapy will be terminated immediately. Evaluation reports, progress reports and any documentation promised by therapist will not be provided until payment has been received in full.
Insurance benefits are a quote and not a guarantee of payment. Benefits, if any, will be assessed by your insurance plan. Upon receipt, claims are subject to eligibility and based on plan provisions and limitations in effect at the time of services rendered. Any and all charges/ balances that are not covered by your insurance company are your responsibility to pay.
Services not covered by insurance:
- Progress reports
- IEP meetings
- Phone calls
- Missed appointments (see cancellation policy below)
These services are billed at the cash rate directly to the patient or guardian.
Missed appointments mean that another patient who was in need of our services could not be seen because time was reserved for you or your child. Please be considerate and conscious of this and reschedule appointments as necessary by contacting your therapist directly or our main line at 360-747-7144. 24 hours notice is required to cancel all appointments. Your account will be charged $50.00 for missed appointments and/ or last minute cancellations. This is a fee that can not be billed to your insurance and is the responsibility of the patient/guardian. Exceptions will be made when weather makes it unsafe to travel and emergencies.
